Liberate Health solves this problem by providing a solution for Healthcare Professionals (HCP). Curated education or “decks” on different treatments and conditions are provided for HCP to review with their patients at point-of-care. This content is personalized in a Patient Visit Record (PVR)and then notification is emailed to each patient for access on our secure web-based portal for them to review.
A “Patient Visit Record” or PVR is a custom digital archive of the conversation between you and your patient at point-of-care. You can create a PVR while educating patients using the condition and treatment “decks” provided in the mobile App. This customization is created leveraging the following capabilities within the App:

Infographic-based content: Infographics, or data visualizations, are leveraged to develop condition and treatment decks to ensure the information is meaningful and memorable to patients. These infographic decks are also created and reviewed by healthcare experts in their field (Key Opinion Leaders) to ensure that each teaching asset is accurate and up-to-date.
Patient Portal: The Liberate Health portal offers patients a secure online environment to access and review their patient visit record after point-of-care. HCP’s also provide curated contextual information that patients can read post visit, ensuring patients are receiving credible information from their providers..

Liberate has been developed for Apple iOS™ only – iPad™ and iPad Mini™. Future phases will include platforms such as Android.
