Why does my practice need Liberate Health?

Research show that patients retain less than 20% of the information they hear from their provider at point-of-care, leading to misinformation, poor adherence to treatment and drug plans, and suboptimal patient outcomes.

Liberate Health solves this problem by providing a solution for Healthcare Professionals (HCP).  Curated education or “decks” on different treatments and conditions are provided for  HCP to review with their patients at point-of-care. This content is personalized in a Patient Visit Record (PVR)and then notification is emailed to each patient for access on our secure web-based portal for them to review.

What is a “Patient Visit Record” and how do I create one using LiberateHealth? 

A “Patient Visit Record” or PVR is a custom digital archive of the conversation between you and your patient at point-of-care. You can create a PVR while educating patients using the condition and treatment “decks” provided in the mobile App.  This customization is created leveraging the following capabilities within the App:

  • Annotation
  • Insert Image
  •  Zoom In
  • Add blank slide
  • Remove slide
  • Add sticky note
  • Add attachments

What makes Liberate Health unique?

Mobile: Liberate has created the first-of-its-kind healthcare App for education at point-of-care.  Liberate believes mobility is critical to driving HCP adoption as Physicians, Physician Assistants, Nurses and Pharmacists begin to use their mobile devices with patients.

Infographic-based content:  Infographics, or data visualizations, are leveraged to develop condition and treatment decks to ensure the information is meaningful and memorable to patients. These infographic decks are also created and reviewed by healthcare experts in their field (Key Opinion Leaders) to ensure that each teaching asset is accurate and up-to-date.

Patient Portal: The Liberate Health portal offers patients a secure online environment to access and review their patient visit record after point-of-care.  HCP’s also provide curated contextual information that patients can read post visit, ensuring patients are receiving credible information from their providers.. 

Is Liberate Health secure? 

Yes, Liberate Health meets all U.S. Healthcare compliance requirements, including those required in the Health Insurance Portability and Accountability Act (HIPAA).

Does the Liberate Health mobile App work on all devices?

Liberate has been developed for Apple iOS™ only – iPad™ and iPad Mini™.  Future phases will include platforms such as Android.

Does Liberate Health integrate with Electronic Medical Record and Electronic Health Record systems?

We are currently developing an integration strategy for Liberate Health.  We understand that this is important to some of our users and expect to be able to integrate with EMR/EHRs in 2014.

Is Liberate Health only available for physicians?

No, Liberate Health is available for all Healthcare Professionals, including Physicians, Physician Assistants, Nurses and Pharmacists.  Each can create a “Patient Visit Record” to send to the patient to access in their Liberate Health portal.

How many Healthcare Professionals are currently using Liberate?

Liberate Health launches  October 2013.  Research studies have demonstrated a significant number of HCP’s “intend to use” – and in one study, 87% of primary care physicians stated they would use in their practice.
